By signing below, I agree that as a volunteer in the Connect: Where Love Never Forgets, Inc., program that I agree to the following conditions:

PHOTO RELEASE:
I give permission and release for PHOTOGRAPHS to be made of myself while engaged in program activities. These photos may be used for publicity/ promotion of Connect Respite Program and for identification purposes.

CONFIDENTIALITY:
All volunteer and participant personal information shared during Respite activities is to be considered confidential and is not to be shared with anyone at any time outside of the Respite program.

ENGAGEMENT:
Ongoing engagement between participants and volunteers is essential to building trust and relationships in order to maximize the Respite experience. Everyone is included on an equal basis and is to be welcomed and supported at all times.

PURPOSE:
Volunteers should look for opportunities for participation to help others or to serve the group throughout the day to help give the participants a sense of purpose and to build self-esteem. Always thank participants for helping and praise them for a job well done.

VOLUNTEER OWNERSHIP:
The success of the Respite experience is due to the shared talents and attributes of the diverse volunteer team. Volunteers are encouraged to share their talents, artistic, musical and other professional and personal experiences and abilities as appropriate. Volunteers are also encouraged to share ideas, suggestions and positive/constructive feedback to continually improve the program.

TEACHING MOMENTS:
There are always unexpected accidents and situations that can provide learning opportunities for everyone. It is important to communicate these situations to the Director or other members of the leadership team to ensure that appropriate changes or interventions are made as necessary.

FLEXIBILITY:
Participants living with cognitive deficiencies can experience changes in mood or behavior from minute to minute, so it is important to remain flexible and calm at all times and to be open to unexpected changes that may occur. The participants’ safety is always our #1 priority.

BACKGROUND CHECK:
I understand that I will be asked to complete a form that allows Connect to complete a background check on me.
